We provide a clinical nurse to follow the care plan built by the patient’s physician. Service offering for Private Insurance – Our PLUS 1 Wellness program is enhanced to screen employee populations. Healthcare costs are skyrocketing because of patients being unaware of their projected risk. We provide a tailor-made solution for beneficiaries, one that utilizes a risk prediction model. Then, a care plan is designed based on the guidance from our clinical health coach. We estimate that approximately 95% of benefit recipients have a preventative component in their plan that is not used correctly.
